Available Settlement Benefits:

Credit Monitoring (All Class Members)

You may enroll in three years of CyEx Financial Shield Complete, which includes:

• Alerts for unauthorized or high‑risk financial activity

• $1 million financial fraud insurance

Out‑of‑Pocket Losses (Up to $3,500)

You may claim documented expenses related to the incident, such as:

• Unreimbursed fraud losses
• Credit report or monitoring fees
• ID replacement costs
• Postage for contacting financial institutions

Proof (receipts, statements) is required.

Lost Time (Up to $80)

You may claim up to four hours at $20/hour for time spent responding to the incident such as password changes, investigating suspicious activity, or researching the breach.

Alternative Cash Payment ($45)

You may choose a one‑time $45 payment.
